I bought the shoulder pillow and cover a couple of weeks ago to try and help stabilize the partial dislocation in my right shoulder. I am 5'3", 130 lbs and this fits between my shoulders well. I can imagine it might not work for bigger people, so sizing might be an additional consideration. I can really feel the stretch in my shoulders the next day, so I wouldn't increase firmness, but that may also be needed for some. It's very comfortable, especially under my neck, once I have it positioned correctly. I've had some very good nights' sleep with it. I

A bit difficult to put on the pillow
It takes a bit of struggling to fit it over the pillow, but I managed. May I suggest adding a zipper? Has a good fit however.

How do you sleep on it? Your other specialty pillows show sleepers using the pillow, so you can see how they are used. There isn't any picture with this pillow that does that, and I can't quite determine how you would lay on it.
